How to make an appointment
Telephone (0118) 9462224 from 8.00am (Mon. to Friday) Urgent cases will always be seen at the next surgery. Every attempt will be made to ensure you are seen on time.
How to obtain a repeat prescription
If you are on authorised medication and require a further supply then this must be requested in writing.
If the prescription is requested in the morning then it will normally be ready for collection by 4pm on the same day. In all other cases it will normally be ready for collection by 4pm on the next working day.
If you would like the prescription posted back to you, then please enclose a SAE with your request.
Alternatively you can email a request to email@example.com
If you have regular medication (or supplies such as diabetic testing equipment) then it is helpful to us if you bring everything with you each time you attend.
Our preference is to see you in surgery on a regular basis and then to give you enough medication to last until you are next seen.
Even if this procedure is not followed we will need to see you from time to time to make sure that your medication is still appropriate.
How to request a home visit
If you need medical help and are too ill to attend surgery then telephone Reading 9462224 to request a home visit. Please provide enough information for us to assess how soon you need to be seen.
What to do if the surgery is closed:
At night and weekends please call 111. You can call 111 when you need medical help fast but it is not a 999 emergency. NHS 111 is a fast and easy way to get the right help, whatever the time.
There is a handrail at the front entrance to the surgery and a ground floor W.C.
At this practice any patient is welcome to see any GP. However the "named accountable "GP for male patients is Dr Harold Chadwick and the "named accountable GP" for female patients is Dr Janet Chadwick.
